Advice from your local drug dealer. Some have good luck with cherry extract capsules. If that doesn't work there are a few prescription drugs (uloric and colchicine) that work to help prevent as well as treat flare ups. If you are over 62 and haven't had your shingles vaccine I strongly suggest you look into it. Many Medicare part d insurances will cover it because they realize an ounce of prevention is worth a pound of cure. Don't wait til,you have a case of shingles to decide if it is worth it. My wife had shingles a couple years back and still has pain issues associated with it.

Wife of a friend suffered shingles, said months of doctor treatments brought no relief. I made her a drawing of the PLANTAIN plant (not to be confused with the banana-like plant). She collected plantain leaves, eating four leaves three or four times a day. Within a week she felt relief and by two weeks healing was well in hand. She said the change was dramatic. Plantain is said to have come from Europe with settlers. Common throughout the east, north to south. Populates lawns which aren’t poisoned, roadsides, trails, the edges of hayfields, places reached by sunlight. Tolerates a range of soils and temperatures. Said to contain beta carotene (A), calcium, ascorbic acid (C), Vit K.
Plantain acts on herpetic ailments which travel nerves and erupt on the skin. If I get into the poison ivy, I’ll eat plantain leaves (oral) and rub crushed leaves (topical) on the affected area. Collected leaves may be air dried and stored in a sealed container out of sunlight and away from ultra violet light. As alcohol conserves herbs, a tincture may be made with dried leaves packed into a mason jar and covered with vodka. As a topical poultice, rubbing alcohol is my preferred vehicle.
The seed of plantain is known as PSYLLIUM, widely distributed as dietary fiber. (Pronounced silly-um.)
The plantain leaf has the shape of a wide spear point. The edge is smooth, without waves or saw teeth. A center vein from stem to tip is flanked most often by two veins on each side, both arc to reunite, or try to unite, at the tip. The side veins do not go to the edge of the leaf. Plantain grows at ground level, each leaf on its own stem. One or two stalks sprout from the center of the plant, coated with hundreds of round seeds----psyllium----the size of birdshot (half the diameter of a BB).
Stopped, recently, for a water break, biking single track trails in mountain hardwoods. Wandered into a grassy clearing, profuse with plantain. Ate a bunch of leaves. Also collect plantain along with dandelion leaves and fiddleheads to throw in salad. Never heard of nor seen a side effect from plantain. Taken orally, the leaf seems to be an immune booster. I've been enjoying Gout for some 25 years. I've had flare ups in the knees, ankle, foot and toes. The doctors say red meat, rich foods and drink cause the flare ups. I've found two triggers so far (asparigus and cranberry juice and a possible third, liverwurst). I work frequently in dehumidified enviroment for steel painting, potable water tanks, fuel tanks and bridges. The best defense is hydration. I try to drink at least a half gallon of water a day to 5 quarts when working in a dry climate (RH < 30.0 %). Gout is not arthritis, but the damage of the acid crystals (sand) in your joints over time will cause it later in life.
If you have a flare up and think it is gout, seek medical attention for prescription drugs to reduce the uric acid level is your body. Everyone has it but when it reaches about 7, it can not stay disolved in fluid i.e. salt or sugar water. The drugs will reduce the level and water will flush it out of your system. When you in pain, organic cherry juice daily may work as a maintenance regime but will do little for immeadiate reduction.
I've been on Allopurinol x 300mg per day for years and still occassionally have flare up when I don't consume at least a half gallon of water per day. For pain during flare ups, i have been perscribed Indomethacin 50mg caps x 3 per day which i occassionally use since it'll jack my BP. If it is a bad flare up and I have a locked ankle or hot toe and can't walk climb scaffolding easily I might double up the first dose of Indomethacin.
How do you know it is gout and not a broken toe. Ice does little to relieve the pain and just the weight of a bed sheet on it causes pain. It may be red and slightly swollen but I've only had minir swelling on a couple of occassions.

Fortunately it only lasts about three days.

Taking my two boys hiking today to a fire tower I rented. This will be my 4 year olds first backpacking trip. It is only a 1.5 miles so if my toe acts up I won't have too suck it up for very long. I got gout a few years ago in my right big toe. If my sheet even touched it, I thought I was going to jump out of my skin! Hot to the touch, and very painful! Turns out I was trying to reduce my cholesterol by taking some supplements, one of which was niacin. After researching gout, I discovered niacin can trigger or cause it. Stopped taking the niacin and after a few days of taking Colchicine, the gout went away. I haven't taken niacin since, nor have I had a recurrence of gout.
