I kept thinking the same thing but also kept thinking it away, the more I thought about it.
As long as it doesn't jam the cylinder...
The string/knot may jam the cylinder if it is too big.
Otherwise, the rim doesn't really do much and that is why there are plastic moon clips and why you can use .45 ACP in dedicated .45 ACP revolver cylinders which are cut to index off the case mouth. The knot is just enough to hold case in place for the hammer strike.
If the knot does not hold and the cartridge is driven into the cylinder, ithe primer may not detonate (and nothing happens) or the primer does go off anyway.
Upon detonation, the rim does nothing. The case is thrust backwards against the frame face.

I've heard of using rubber O-Rings. But, haven't tried it.

My brother tried in a Rossi Ranch Hand .45 Colt levergun for function (did not actually shoot a live round) and he said it works single fed.
It extracts too apparently.
