It could be many things;
Extractor tuning
Break over geometry (feed ramp to chamber radius)
Ramped barrel hanging up and not dropping down all the way or, needs to drop down more due to tolerance stacking
Mag release point too low.
If the problem persists after the trip back to the mother ship a time or two, it just may be as Mr Bradshaw said and be a complete cosmic aligning of the stars abd be a lemon. With production tolerances as they are, the individual components can be within spec but at the far side of each other and not work together as a sub-system. It will take some sub-system diagnostics anf measuring and meaduring to fix the problem. First trip back to the mother ship will get the post production mods that tend yo fix 98% of the problems by a novice to intermeadiate person. If a pistol comes back a second time, an experianced plumber will have to look at each sub-system and parts associayed with that systen to diagnose and fix the problem.
